Arthur O'Connell signs an "Employee's Withholding Exemption Certificate" in 1964.

Partly Printed Document Signed: "Arthur O'Connell", 7¼x3¼ computer punch card. Beverly Hills, California, 1964 June 15. Form W-4. Headed: "Employee's Withholding Exemption Certificate". O'Connell's name, social security number (blacked out on scan) and address printed at upper portion. Two exemptions are claimed. In the year he signed this document, O'Connell appeared in three feature films, Kissin' Cousins, 7 Faces of Dr. Lao and Your Cheatin' Heart, and he appeared on epsiodes of the TV series Breaking Point, Arrest and Trial, The Greatest Show on Earth, Burke's Law and Summer Playhouse. Veteran character actor Arthur O'Connell (1908-1981) was nominated for Best Supporting Actor Academy Awards for Picnic (1955) and Anatomy of a Murder (1959). O'Connell, who made his stage debut in the mid-1930s, appeared in his first film in 1938. His film credits include Citizen Kane (1941), The Man in the Gray Flannel Suit (1956), Bus Stop (1956), Gidget (1959), Operation Petticoat (1959), Cimarron (1960), The Great Imposter (1961), Pocketful of Miracles (1961), Follow That Dream (1962), The Silencers (1966), Fantastic Voyage (1966), The Poseidon Adventure (1972) and The Hiding Place (1975). O'Connell, who appeared on a number of early TV anthologies, was also a regular performer in two series, Mr. Peepers (1953-1954) and The Second Hundred Years (1967-1968). He also mad guest appearances on a long list of TV series through 1975. Lightly scuffed at upper portion, affecting two lines of imprinted text (all legible). Staple holes at blank left margin. Fine condition.
