With 6¼x3¼ initiation fee receipt carbon copy with perforated left edge stapled at top edge. July 26, 1954. Membership form for the American Federation of Television and Radio Artists. According to this form, Peoples was scheduled to appear in a 1954 episode of The Red Skelton Show.
