Document signed "David M Whorf" in blue ink. Blue ink notations in unknown hand. 2 pages, 6½x9¼, 1 sheet, front and verso. With 6½x3¼ initiation fee receipt carbon copy with perforated left edge stapled at top edge. Nov. 4, 1958. Membership form for the American Federation of Television and Radio Artists. In 1958, Whorf had appearances in numerous TV shows, including Man with a Camera, Have Gun, Will Travel, Tombstone Territory, The Adventures of Jim Bowie and Gunsmoke. Whorf, born in 1934, appeared in over 20 movies and TV shows between 1948 and 1965, with starring roles in Twist All Night and One Way Wahini, before stepping behind the camera to become a director and producer. He was a director on Spencer: For Hire (1986-1988), a producer on The Brady Bunch (1969-1970) and an assistant director on numerous movies and TV shows, including Caddyshack (1980) and Porky's Revenge (1985), the mini-series A Woman Named Jackie (1991) and the TV series Batman (1966-1968), Land of the Giants (1969), Cannon (1971-1975), The Streets of San Francisco (1975-1977), Heaven and Hell: North & South, Book III (1994) and CSI (2001). Whorf stepped back in front of the camera once in 1996 for the TV movie Christmas Every Day. Lightly toned, soiled and creased. Form has lightly tears in right edge and is discolored on verso (no show-through). Receipt has carbon copy marks and was folded once and unfolded. Otherwise in fine condition.
