Document signed "Don Pulford" in lead pencil. Lead pencil and blue ink notations in unknown hand. 2 pages, 6x8½, 1 sheet, front and verso. With 6¼x3¼ initiation fee receipt carbon copy with perforated left edge stapled at top edge. Oct. 11, 1958. Membership form for the American Federation of Television and Radio Artists. Actor and stuntman Pulford, born in 1936, has been a stuntman in almost 60 films and TV shows between 1972 and 2001. Most notably, he's been a stunt double for William Shatner in three Star Trek movies (Star Trek V: The Final Frontier, 1989; Star Trek VI: The Undiscovered Country, 1991; and Star Trek: Generations, 1994) and for Nick Nolte in three of his movies (Three Fugitives, 1989; Cape Fear, 1991; and I Love Trouble, 1994). Lightly toned and creased. Form has light tear in right edge and light dents and nicks at top edge and was folded twice and unfolded. Receipt carbon copy marks and was folded once and unfolded. Otherwise in fine condition.
