Document Signed: "Francis Xavier Bushman" and "Francis X Bushman", 1p, 8½x5½. Hollywood, California, 1960 November 22. Headed: "Twentieth Century-Fox Film Corporation/Employee's Personnel Statement. Bushman has filled in his social security number, name, address, occupation and emergency contact ("Mrs. Bushman"), checked boxes for sex, race and marital status and dated the form "Nov 22". The year before he filled in this form, the actor, who was 77 year of age when he signed this document, had appeared in 1961's The Phantom Planet. He would next appear on the big screen in Peer Gynt (1965). A former sculptor's model, Francis Xavier Bushman (1883-1966) had a beautiful physique which, combined with his handsome looks, propelled him to stardom in pre-20s silents. Known as "the handsomest man in the world" and the first film actor to be called "King of the Movies", he starred in 115 silent films between 1911 and 1914, including his most famous role, Messala in Ben-Hur (1925). Bushman made more than a million dollars a year during the silent era, but lost his fortune in the Crash of 1929. After 1930, he appeared in feature films sporadically, but became a star of radio soap operas. Bushman, who later appeared in David and Bathsheba (1951), Sabrina (1954), The Story of Mankind (1957) and Twelve to the Moon (1960), made his last film, The Ghost in the Invisible Bikini, in 1966, the year of his death. "Bushman" on name line has been crossed out and "BUSHMAN" printed in pencil above it. Lightly creased at upper corners. Ink notes (unknown hand) at upper left margin, red ink notes (unknown hand) on 1 line and at upper blank area and pencil notes (unknown hand) at upper right margin and in imprinted box. 2 file holes at upper margin.
