Academy Award nominee Garrett Lewis signs an AFTRA application in 1960
Document signed "Garrett Lewis" in blue ink. Blue and red ink notations in unknown hand. 2 pages, 6x8½, 1 sheet, front and verso, with 6½x3¼ initiation fee receipt carbon copy with perforated left edge stapled at top edge. 1960 January 13. Membership form for the American Federation of Television and Radio Artists as a dancer, singer. Garrett Lewis, first joined AFTRA as a singer/dancer in 1960. After appearances on Shirley Temple's Storybook (1961), The Red Skelton Show (1963) and a few others, he found his real talent as a Set Designer - he was nominated four times for an Academy Award for Set Decoration: Beaches (1989), Glory (1990), Hook (1992), Dracula (1993). An Emmy nomination in 2000 for Outstanding Art Direction for a Miniseries, Movie or Special Emmy was shared with Charles Wood and Mark Zuelzke for their work on Geppetto. Form is lightly creased. Receipt has notch at upper right edge and carbon smudges. Otherwise, fine condition.
