GEORGE J. "BART" BURNS. Partly Printed DS: "George J 'Bart' Burns" on verso, 7¼x3¼ card. North Hollywood, California, 1965 November 12. Headed: "Personnel Record" on front and "Employee's Withholding Exemption Certificate" on verso. Burns' personal information, including date of birth (3/31/1923), birthplace (New York City, N.Y.), address, marital status (married to Fern Frances Burns) and social security number (blacked out on scan) filled out on front. Burns has claimed six exemptions on verso, which he has dated at lower left. In the year he signed this document, Burns appeared in an episode of the TV series, Convoy. George J. "Bart" Burns appeared in a number of feature films, including Between Heaven and Hell (1956), Fear Strikes Out (1957), Seven Days in May (1964), The Iceman Cometh (1973), Frances (1982) and Legal Eagles (1986). His television credits include several made-for-TV movies, including Call to Danger (1973) and Helter Skelter (1976), and guest starring appearances on shows from Goodyear Television Playhouse (1953) to Lou Grant (1979). 1½x¼-inch portion cut away at upper left on front, removing portion of date on verso. Two file holes, one at the "Ba" of Bart and one nicks the "6" of Burns' date.
