Partly Printed Document filled out and signed three times: "Hermione Gingold" in pencil, 7x3¼ card. No place, no date. Department of the Treasury-Internal Revenue Service Employee's Withholding Exemption Certificate (Form W-4; revised 6/77). Front is headed: "Personnel Record". Gingold has written her personal information, including her social security number and address (on both front and verso), telephone number, date of birth, occupation and emergency contact (information blacked out on scan is present on document). An English Shakespearean actress who shifted focus in mid-career to stage and film musical comedy, Gingold (1897-1987) won a Golden Globe award as Best Supporting Actress for Gigi (1958) and was nominated for another for The Music Man (1962). She was nominated for a Tony for A Little Night Music (1973), reprising the role in the 1977 film. Slightly creased. File hole at upper margin nicks the "G" of Gingold on verso. Staple holes at upper left corner, stamp at imprinted area on verso, pencil and ink marks notes (unknown hand). Overall, fine condition.
