front and verso. No place, 1959 October 30. Headed: "Certificate
of Nonresidence Claim for Exemptions". Form 591 for 1959. Warden's New York City
address typed on front, his California address and the name and address of his
employer, Mervyn Leroy Productions, Inc., typed on verso. The actor has claimed
a $3,000 exemption as "Married". In 1959, Warden appeared in two feature
films, The Sound and the Fury and That Kind of Woman, and he was
seen on TV as a guest star on Playhouse 90, Bonanza, Five
Fingers, The Lawless Years, The Twilight Zone and The
Untouchables. A former boxer, bouncer and lifeguard, Jack Warden
(1920-2006), born John Lebzelter, was a frequent screen presence for half a
century. He received Best Supporting Actor Oscar nominations for
Shampoo (1975) and Heaven Can Wait (1978). Other prominent
film roles include Twelve Angry Men (1957), Being There (1979) and
Bullets Over Broadway (1994). Warden, who won an Emmy Award for
the TV movie, Brian's Song (1969), also starred in several series,
including The Wackiest Ship in the Army (1965-1966), NYPD
(1967-1969), The Bad News Bears (1979-1980) and Crazy Like a Fox
(1984-1986). Slightly creased. Two file holes at upper margin. Fine
condition.
