Meal allowance for "Breakfast" on March 31, 1962 through "Dinner" on April 1, 1962. The total for two breakfasts, a lunch and three dinners was $24.50. Noted to be for Production No. 15226. Also signed by the Assistant Production Manager. Probably for the 1962 film, Girls! Girls! Girls!, in which Slate co-starred as Wesley Johnson opposite Elvis Presley and Stella Stevens. In 1962, Slate was also seen on the small screen on episodes of Perry Mason, Dr. Kildare, Alfred Hitchcock Presents, Route 66 and The Defenders. Stage, screen and television actor Jeremy Slate (1926-2006) made his film debut in North by Northwest (1959) and went on to appear in such features as Girls! Girls! Girls! (1962), Wives and Lovers (1963), The Sons of Katie Elder (1965), True Grit (1969) and The Lawnmower Man (1992). After making his TV debut in 1959, Slate would appear on nearly 100 television shows, including recurring roles on Men Into Space (1959-1960), The Aquanauts (1960-1961), Run for Your Life (1965-1966), One Life to Live (1979-1987, as Chuck Wilson, his longest running role) and The Guiding Light (1985). The multitalented Slate also appeared on Broadway, wrote the screenplay for the 1969 film, Hell's Angels '69 and wrote lyrics for a number of songs, including hits for Tex Ritter and Glen Campbell. Slate was married to actress Tammy Grimes from 1966-1967. Lightly creased. Cancellation holes at right margin, not at signature, and lower margin, which is also stamped. Staple holes at upper blank margin. Fine condition.
