ink stamp on verso. 2 pages, 6x8½, 1 sheet, front and verso, with 6¼x3¼ initiation fee receipt
carbon copy stapled at top edge. Feb. 3, 1956. Membership form for the American Federation
of Television and Radio Artists. American singer and composer Mann (1928-2014) is
probably best known as leader of the Johnny Mann Singers, a vocal group that hosted its
own TV show, Stand Up and Cheer, in 1971. Known for their clean-cut music and dress,
the Johnny Mann Singers earned Grammies for their album Great Band With Great
Voices in 1961 and their single Up-Up and Away in 1967. Mann was also music director
for the original Alvin and the Chipmunks TV show (The Alvin Show, 1961) and voiced
Alvin. Lightly toned and creased. Form is lightly torn at left edge. Receipt has carbon copy
marks and was folded once and unfolded. Otherwise in fine condition. This image has been
censored to conceal personal information depicted in the image. The actual document
does not have these concealing marks.
