ink notations in unknown hand. 2 pages, 6½x9, 1 sheet, front and verso. With
6¼x3¼ initiation fee receipt carbon copy with perforated left edge stapled at
top edge. 1958 September 15 Membership form for the American Federation
of Television and Radio Artists as an actress. Joy Windsor (Ellis) had a
credited career that began in 1928 with Life's a Stage and ended with
Come on Seven (1956), mostly bit parts and uncredited scenes. Form
lightly creased and soiled at corners. Vertical fold crease. Receipt is creased
along top and left edges, light discoloration at edges and carbon copy marks.
Otherwise, fine condition.
