notations in unknown hand. 2 pages, 6½x9, 1 sheet, front and verso. With 6½x3¼
initiation fee receipt carbon copy with perforated left edge stapled at top
edge. Membership form for the American Federation of Television and Radio
Artists. According to this form, Angleo was scheduled to appear in a 1956
episode of Matinee Theatre. Smith appeared as Tom Mason in the
Alfred Hitchcock Presents episode The Case of Mr. Pelham (1955),
as Colonel Blight in the Studio 57 episode Night Tune (1955), as
Professor Lamont in the Highway Patrol episode Desert Copter
(1956), as Mayor Archer in the Circus Boy episode The Cub Reporter
(1956), as Mr. Botkin in the Gunsmoke episode The Man Who Would Be
Marshal (1957), as Wesley Cavendish in Ghost of Dragstrip Hollow (1959) and
as a pirate in the 1960 movie Peter Pan. Lightly toned and creased.
Signature touches notations. Form has light tears in left edge. Receipt has
carbon copy marks and was folded once and unfolded. Otherwise in fine
condition.
