Document signed "Larry Maldonado" in blue ink. Blue ink notations in unknown hand. 2 pages, 6¼x9, 1 sheet, front and verso. With 6¼x3¼ initiation fee receipt carbon copy with perforated left edge stapled at top edge. May 20, 1954. Membership form for the American Federation of Television and Radio Artists. Maldonado joined AFTRA as a dancer but made an appearance in The Untouchables (1960) as Fabiano. At the time of this document, Maldonado was scheduled to appear on an episode of The Jack Benny Show (1950-1965) - a TV version of comedian Jack Benny's radio show, which ran under different names and sponsors from 1932 to 1955. A show-within-a-show, Benny and show regulars Don Wilson, and Dennis Day (not to mention Benny's "chauffeur" Rochester, played by Eddie Anderson) opened the show with jokes and a song, then played themselves in a short skit, play or movie satire. The Jack Benny Show earned 20 Emmy nominations during its run and won seven, including two for Benny himself and one for Best Comedy Series. Lightly toned and creased. Form has light tears in right edge and top right corner and is lightly discolored on verso (no show-through). Receipt has carbon copy marks and was folded once and unfolded. Otherwise in fine condition.
