AFTRA membership application signed by actor Lee Kolima in 1967
Document double signed: "Lee Kolima" and "Chas Zalopany" in blue ink. Blue ink notations in unknown hand. 2 pages, 6x8¾, 1 sheet, front and verso. With 6¼x3¼ initiation fee receipt carbon copy with perforated left edge stapled at top edge. 11 December 1967. Membership form for the American Federation of Television and Radio Artists as an actor. American actor Lee Kolima (1920-1995) also worked under the name Charles Zalopany. A bit actor in the 1960s he appeared uncredited in films such as 7 Women (1966), The King's Pirate (1967) and P.J. (1968). His last film was in 1984 as Nicky in Cannonball Run II. Between 1965 and 1975 he hit many of the popular TV series such as I Spy (1965-1966), Get Smart (1965-1966), The Monkees (1966-1968), and The Wild Wild West (1968). In 1967, Kolima appeared on both The Red Skelton Show and The Jonahtna Winters Show. Lightly toned and creased. Receipt has creases at top edge and carbon copy marks. Otherwise, fine condition.
