Document signed "Mary Mitchel" and "Mary Patton" in black ink. Blue and red ink notations in unknown hand. 2 pages, 6x8½, 1 sheet, front and verso. 1964 August 5. Membership form for the American Federation of Television and Radio Artists. A Hollywood, California native, Mary Mitchel (born 1940), began her acting on stage while attending UCLA. Throughout the 1960s she worked as an actress on television programs such as Leave it to Beaver (1961), The May Loves of Dobie Gillis (1961-1962), Disneyland (1962) and Hogan's Heroes (1966-1967). She later worked behind the scenes in various capacities on such films as The Godfather (1972), Bram Stoker's Dracula (1992) and other Francis Ford Coppola projects - Mitchel, Coppola and her husband Bart Patton all attended UCLA together. Lightly creased. Staple holes at top left and staple at top right corner. 2 horizontal fold creases. Otherwise, fine condition.
