Document signed three times: "Michael Ansara", 1 page, 8½x5. He has also handprinted his own name ("Michael George Ansara") and written his wife's name ("Jean Ansara"). May 1, 1954. Ansara has filled out eleven lines of personal information in pencil. Michael and Jean Ansara were divorced in 1956. He was married to actress Barbara Eden from 1958-1974 (divorced). In films from 1944, Michael Ansara (1922-2013) starred as Cochise in the TV series Broken Arrow (1956-1958). Although he has written that he was born in Lowell, Mass. on April 15, 1922, the truth is he was born on that day in Syria and moved with his parents to Lowell when he was two-years-old. Ansara has often been cast as American Indians, Mexicans and Star Trek Klingons. He has made dozens of guest appearances on TV series, and does voice work for animated cartoons. Pencil notes (unknown hand) and small stamp mark. Vertical fold passes through "George" and also [Jean] "Ansara". Otherwise, fine condition.
