Document double signed: "Ned Miller" and "Nathan Miller" in ink. Blue ink notations in unknown hand. 2 pages, 6x8¾, 1 sheet, front and verso. With 6½x3¼ initiation fee receipt carbon copy with perforated left edge stapled at top edge. September 4, 1962. Membership form for the American Federation of Television and Radio Artists as an actor. Actor-performer Ned Miller spent most of his career with the Jack Benny Program, appearing in 22 episodes from 1961-1965. The Jack Benny Show (1950-1965) was a TV version of comedian Jack Benny's radio show, which ran under different names and sponsors from 1932 to 1955. A show-within-a-show, Benny and show regulars Don Wilson, and Dennis Day (not to mention Benny's "chauffeur" Rochester, played by Eddie Anderson) opened the show with jokes and a song, then played themselves in a short skit, play or movie satire. The Jack Benny Show earned 20 Emmy nominations during its run and won seven, including two for Benny himself and one for Best Comedy Series. In 1984, Miller was wrote the screenplay Scarab. Form is lightly creased and toned at edges and corners. Receipt has a fold crease at top edge and carbon copy marks. Otherwise, fine condition.
