Partly Printed Document Signed: "Raymond Burr", 1p, 8½x12¼. Beverly Hills, California, 1989 October 27. Medical Certificate filled out and signed by his doctor, Selvyn Bleafer, required by Viacom Productions for Perry Mason #14-"Paris Paradox". Dr. Bleafer describes Perry Mason star RAYMOND BURR's general appearance as "Healthy", height "6'1"," weight "300", temperature "98", blood pressure "140/80" and pulse "72". The insurance company has accepted the doctor's report subject to "overage surcharge $5,000." Burr's date of birth is listed as May 21, 1917, so he was 72 at the time of this examination. Actor Raymond Burr (1917-1993) starred as Los Angeles lawyer Perry Mason from 1957-1966 and then as San Francisco Chief of Detectives Robert T. Ironside from 1967-1975. He revived his lawyer role in a two-hour TV movie Perry Mason Returns in 1985. It was the second highest rated TV movie that season and led to three or four such films during each successive season (one was 1987's Paris Paradox) until Burr's death in 1993 at the age of 76. The doctor's name, signature and insurance company name have been crossed out in black. Upper left corner cut away, small stain in lower left corner, bottom portion (at perforated line) has been reaffixed to sheet with tape. Otherwise, fine condition.
