Questionnaire signed twice: "Shannon Miller" on front and verso, 7x4 printed questionnaire. American gymnast and gold medal Olympian Shannon Miller was born on March 10, 1977. She is widely considered to be one of America's finest gymnasts. In 1993 and 1994 she was the World All-Around champion. She was the most successful athlete by medal count at the 1992 Olympic Games, winning two silver and three bronze medals. She then brought her grace and talent to the 1996 Olympic Games in Atlanta and won two gold medals. In the 1995 Pan American Games she won four gold medals and was the all-around champion. Miller ranks as the most decorated gymnast in U.S. history. She is in both the USA Gymnastics Hall of fame and the US Olympic Hall of Fame. Ink note (unknown hand) on verso.
