Includes receipt noting the membership dues paid by the performer. Bandleader Ted Lewis (1890-1971) maintained an active recording, radio, movie, television and concert career for 50 years (1917-1967). His hit songs included "Some Of These Days" and his trademark "When My Baby Smiles At Me". Note taped to front page. Corners creased. Otherwise, fine condition.
