Filled out for Twentieth Century-Fox Film Corp. This is a Form 591 tax exemption form that Bosley filled out while he was employed at Twentieth Century-Fox. He was forced to scratch out the head of household exemption later. After making his film debut in 1963's Love With the Proper Stranger, Bosley appeared in several other feature films, including Yours, Mine and Ours (1968), Gus (1976) and The Fallen Ones (2005). Bosley, who won the 1960 Tony Award for Fiorello, also originated the role of Belle's father in the Broadway production of Beauty and the Beast. Lightly toned and creased. Two binder holes at top.
