Annotated document signed "Vera Miles" in purple ink. 2 pages, 7¾x3¼, one sheet folded, front and verso. Oct. 26, 1967. U. S. Treasury Department Form W-4, Employee's Withholding Exemption Certificate, with her address in Sherman Oaks, California. Miles wrote on this form that she was married and had no exemptions. At the time, she was married to her third and last husband, actor Keith Larsen; they divorced three years after this document was signed. Miles had three children from previous marriages and would have a fourth with Larsen the year of their divorce. Miles, born Vera Ralston in Boise City, Oklahoma in 1929, proved adept at playing hysterical or mentally imbalanced women. A favorite of director Alfred Hitchcock, her film roles include The Wrong Man and The Searchers (1956), Psycho (1960), The Man Who Shot Liberty Valance (1962) and Separate Lives (1995). Miles guest starred in many TV series and pilots through the 1980s. Lightly toned. Staple holes in upper left corner. Otherwise in fine condition.
