Document signed "Francis Ravel" twice in blue ink. Blue ink and lead pencil notations in unknown hand and, on verso, red ink stamp. 4 pages, 1 sheet at 6½x9, 1 sheet at 6x8½, front and verso. With 6¼x3¼ initiation fee receipt carbon copy with perforated left edge stapled at top edge. Sept. 5, 1961. Membership forms for the American Federation of Television and Radio Artists. According to these form, Ravel was scheduled to appear in a 1956 episode of Climax!. Ravel appeared in an uncredited role as a rug peddler in Action in Arabia (1944), in an uncredited role as a waiter in Pan-Americana (1945), as a French guard in the I Love Lucy episode Lucy's Bicycle Trip (1956), in an uncredited role as a French civilian in Gaby (1956), in an uncredited role as a matire d' in The Power and the Prize (1956), in an uncredited role as a reporter in Ten Thousand Bedrooms (1957), in an uncredited role as a French mechanic in Tip on a Dead Jockey (1957), in an uncredited role as a French stage manager in Les Girls (1957), in an uncredited role as a French gate guard in Lafayette Escadrille (1958), in an uncredited role as a waiter in The 3rd Voice (1960), in an uncredited role in A New Kind of Love (1963), as a "man" in the Combat! episode Ambush (1963), in an uncredited role as Francois in Viva Las Vegas (1964) and in an uncredited role as a waiter in Looking for Love (1964). Lightly toned and creased. Signature touches notations. Top form has light tear in right edge and light nick in top right corner. Receipt has light dents and nicks in top edge and carbon copy marks and was folded once and unfolded. Otherwise in fine condition.
