With 6½x3¼ initiation fee receipt carbon copy with perforated left edge stapled at top edge. June 3, 1960. Membership form for the American Federation of Television and Radio Artists. According to this form, Taylor was scheduled to appear in a 1960 episode of Chevy Mystery Theatre.
